Description of Services:
In-home comprehensive geriatric assessment by clinician and/or geriatric physician to address cognitive, physical or functional changes * this service is provided for home bound seniors who are physically unable to attend a clinic and reside in the NYGH catchment area or seniors experiencing home safety concerns
