Description of Services:
Outpatient specialized geriatric services providing older adults with comprehensive assessments in a clinic setting (treatment planning, education and referrals to community and healthcare resources) * home visits available for homebound seniors or for seniors experiencing home safety concerns * Note: the team is not primary care providers and provide a consultative service only
