Description of Services:
Supports for older adults, and adults with disabilities * helps people remain independent in their own homes for as long as possible
  •  
  • Dining for Seniors Group meals and social activities * free blood pressure clinics
  •  
  • Exercise and Wellness Focuses on seniors maintaining and improving their overall health
  •  
  • Foot Care Basic and advanced foot care provided by a nurses certified in advanced foot care management
  •  
  • Friendly Visiting and Telephone Reassurance A support program that matches a volunteer with a senior who is isolated, lonely or could use a friend * the client and volunteer meet on a one-to-one basis and visit in the client’s home * these regular visits can include a telephone conversation, or enjoying activities outside the home
  •  
  • Home At Last Program Assists older adults with transitioning from hospital to home
  •  
  • Home Help Assistance with household chores and daily tasks
  •  
  • Meals on Wheels Hot meals delivered Mon-Fri between 11:30am-1pm * special diet types available

  • Personal Support and Homemaking Services Assistance older people and people with disabilities who are not eligible for Ontario Health atHome South West area and need help with daily chores, such as laundry, light housekeeping, and home maintenance activities * home help visits weekly, bi-weekly, monthly, or seasonally

  • Transportation - EasyRide Non-emergency transportation to local medical appointments and regular activities such as shopping, errands, and social activities * volunteer drivers * wheelchair accessible vans, cars, and taxis
