Description of Services:
The program improves access to timely diagnostic testing and a coordinated approach to patient care supported by a multidisciplinary team of radiologists, surgeons and other key healthcare providers * patient navigator helps coordinate and book all patient's appointments, and provide patients with support and education * staff will inform the family doctor of the results and appointments, and keep them up-to-date about patient's care * provides treatment and care of patients who have been diagnosed with colon or rectal cancer
