Description of Services:

Diabetes nurse educators and registered dietitians offer self-management education and support for people living with prediabetes and Type 2 diabetes. Available to those newly diagnosed or living with these conditions for years, including people starting or already using insulin.

One-on-one appointments (by healthcare provider referral) and group sessions available.
