Adults 18 years and older with a BMI of at least 35, or a BMI between 30 and 35 and a comorbidity of diabetes or hypertension

Hospital-based outpatient program providing medical or surgical treatment for obesity in adult patients * services provided by nurses, registered dietitians, social workers, occupational therapists, psychologists, internists, and surgeons
Progression level of rehabilitative care for patients working to restore or maximize functionality
