Description of Services:
Allows patients to perform dialysis in their own home and offers options regarding type of dialysis * multi-disciplinary team provides visits, training and ongoing support outside regular clinic hours to assist patients to be successful at home * dialysis equipment and supplies are designed specifically for home dialysis and are supplied to patients at no cost * also partner with community agencies to provide patients with home care as needed
