Description of Services:

Outpatient Services

Services offered in clinic setting and in the home when patients are no longer physically able to come to the clinic

complex pain and symptom management
support with establishing goals of care and advanced care planning
support with addressing one’s physical, spiritual, emotional, social and informational needs
support for patients and families facing difficult end of life decisions such as withdrawal of treatment
procedures such as paracentesis and blood transfusions
seamless transition from inpatient care and direct admission wherever possible
