Hospital-based outpatient rehabilitation for people who have had a kidney transplant * services provided by nephrologists, surgeons, psychiatrists/psychiatric nurses, and post-transplant coordinators
Progression and Maintenance levels of rehabilitative care for patients working to restore or maximize functionality, or prevent functional decline or injury, or to maintain functionality
