Eligibility / Target Population
Individuals aged 16 years and older * live outside the Greater Toronto Area * be able to manage your own personal care and medications * be able to attend appointments independently * be able to walk up and down at least one flight of stairs without assistance * be actively receiving cancer treatment at Princess Margaret Cancer Centre or Sunnybrook Odette Cancer Centre
