Description of Services:

Services and programs to meet the needs of youth and young adults with cerebral palsy or acquired brain injury as they transition from pediatric to adult rehabilitation services * provides a single point of access to specialized rehabilitation while helping patients learn how to manage their own care and navigate the adult system

Services may include:

nutrition counselling
spasticity management
pain management
speech language therapy consultation
mobility assessment and supports consultation
consultation around exercise programs
short-term supportive counselling
independent living skills development
