Eligibility / Target Population

Program cares for medically stable clients, 18 years or older who have a demonstration of restorative potential and activity tolerance with therapy, are motivated to participate and agreeable to admission to restorative program, understands and agrees that the goal is a safe discharge to the community

Description of Services:

Provides restorative care for those individuals who no longer require services in acute care hospital, and because of their care needs, are temporarily unable to return to the community.

A Physician and/or Nurse Practitioner will be assigned to you on admission to monitor your progress and attend to any medical needs during your stay.
