Description of Services:
  • Operates a primary care medical clinic consisting of 28 family doctors
  • Provides access to other health care professionals within and outside of the Northern Clinic
  • Coordinates documentation if patient receives care from a member of the team
    • Offers primary health care including but not limited to:
    • Chronic disease education and prevention
    • Disease management
    • Health promotion
    • Rehabilitation
    Telephone Health Advisory Service
    • Provides access to a Registered Nurse through Health Connect Ontario (formerly Telehealth Ontario) who provides health advice and general health information
