Community navigation is the process of connecting citizens to support structures within public services, social services, healthcare or the wider community. It involves outreach, education, and technical assistance to increase awareness of, and participation in, local programs. When people come to your organization, they may have additional or complex needs that may require services and programs from two or more agencies. We invite all those who navigate on behalf of clients to join the Community of Practice Navigation Portal. The Community of Practice Navigation Portal will allow service providers to connect, share, and collaborate to best support their clients.
