Looking for a Health Care Provider? Here are a few options:
First, register for Health Care Connect, a service that helps connect community members to a doctor or nurse practitioner in their community who is accepting new patients. You can register online or call 1-800-445-1822 for assistance. Register yourself and/or your whole family for Health Care Connect, and a care connector will search for a doctor or nurse practitioner who is accepting new patients in the community.
While you are waiting, apply for care through a local Community Health Centre, Family Health Team, or inquire with a local doctor or nurse practitioner to see if they are accepting new patients.

What if I would like to change primary care providers?
If you don't feel confident in your primary care provider and would like to change to another, ask to be taken off her or his patient list. Then contact Health Care Connect at 1-800-445-1822 to find a new provider.
Reporting an incident
In a case where you believe your provider acted unprofessionally, you can file a complaint with the College of Physicians and Surgeons of Ontario by calling 1-800-268-7096 ext 629.
