What to Expect from Home Care


Home and Community care is more than just help in your home. Services may include:
 
  • Nursing
  • Physiotherapy
  • Occupational therapy
  • Speech language therapy
  • Social work
  • Support for healthy eating
  • Home care supplies, such as dressings and walking aids
  • Personal care
  • Homemaking
  • School Support 

There are many reasons you might need some additional care and support to stay safely in your own home. Often, it’s after a hospital stay, as you’re recovering from illness, injury or surgery. Sometimes it’s because you are now living with long-term disabilities or the effects of aging.

If you qualify, the Ontario government pays for a wide range of services in your home and community.

Home and Community Care Support Services - Erie-St. Clair is responsible for deciding who receives care in this region, the level of care you need, and for how long.

Starting the process
 
  1. Call  Home and Community Care Support Services - Erie-St. Clair

You will be introduced to a Care Coordinator. She or he will determine if you qualify for services.

If you don’t qualify, you may be able to receive services through community agencies, which may require a co-payment. You can arrange and pay for services through a private company. If you are having trouble covering the cost of services, make sure to ask about qualifying for subsidy. You can arrange and pay for services through a private company. Home and Community Care Support Services can help you find services in your community.

2. Make a plan
Your Care Coordinator will need to visit your home to assess your health. She or he will create a customized home care plan that meets your specific needs. If your needs change, your Case Manager can reassess your health and adjust your plan of care.


3. Apply for care
If you qualify for government-funded care, Home and Community Care Support Services will coordinate your application and select the provider for you.
